Reducing Fibrosis in Lymphedema

Lymphedema treatment often focuses on one obvious problem: swelling. But ongoing lymphatic decongestion can create damage to tissues and create further swelling.

Karen Ashforth, MS, OTR/L, CLT-LANA, is an occupational therapist and lymphedema expert who specializes in fibrosis. She’s conducted research showing how fibrosis treatment can improve comfort, movement, and long-term symptom control. Her breast cancer-related lymphedema studies also show why success involves more than just reducing swelling.

What Is Fibrosis in Lymphedema?

Fibrosis develops when chronic swelling and inflammation from lymphedema change tissue structure. “It’s like the body is making its own Jell-O,” Ashforth says. Protein-rich lymph fluid thickens the lymph and can attract additional fatty tissue.

Over time, fibrotic tissue may:

  • Become firmer, less flexible, and more extensive
  • Build up fatty tissue
  • Interfere with healthy lymph flow
  • Make swelling harder to reduce

Ashforth says that fibrotic changes occur at all lymphedema stages, including before swelling becomes obvious. Earlier treatment can limit progression, but even long-standing fibrosis may still respond to targeted care.

Why Does Fibrosis Make Lymphedema Harder to Control?

Fibrosis does more than change how your tissue feels. Hardened tissue can interfere with lymph pathways and make fluid harder to move. In later stages, fibrotic and fatty changes can make those drainage pathways even less effective.

Ashforth describes a vicious cycle of inflammation, fibrosis, swelling, and impaired lymph drainage. Moving fluid remains important, but it may not address hardened tissue that blocks healthy circulation. Treating fibrosis can help create better conditions for lymph to move.

How Fibrosis Changes What Treatment Success Looks Like

Swelling may make the area feel a bit like a full water balloon. When treatment moves that fluid, the tissue often becomes looser and more flexible. “But fibrosis still remains because the tissue itself hasn’t changed,” Ashforth explains.

You may notice:

  • Increasing firmness, tightness, or hardness
  • Pain or tenderness
  • More difficulty moving
  • Swelling that doesn’t respond as well to treatment

That difference also changes how you and your therapist may measure your progress. Softer tissue, less pain, and easier movement can matter just as much as smaller measurements. In one of Ashforth’s studies, breast tissue became softer, and pain levels decreased.

How Breast Cancer-Related Lymphedema Can Affect the Upper Body

Breast cancer-related lymphedema can involve fibrosis in several parts of the upper body, including the:

  • Arm
  • Armpit
  • Breast
  • Chest
  • Hand

Swelling, surgery, radiation, and tight tissue bands (cording) are all considered types of fibrosis. These fibrotic tissue changes can:

  • Cause hardness
  • Interfere with lymph drainage
  • Restrict movement
  • Trigger pain

“Fibrosis can act like a barricade between swelling and the lymph nodes that help drain fluid,” Ashforth explains.

In one of Ashforth’s case studies, patients still had pain, swelling, and fibrosis after complete decongestive therapy (CDT) and home care. After adding Lympha Press® treatment to the arm, armpit, breast, and chest for three to four weeks, patients experienced:

  • Less arm and chest swelling
  • Reduced pain and tightness

How Clinicians Combine Treatments to Soften Fibrosis

CDT combines manual lymph drainage, compression, exercise, and skin care. Fibrosis may require additional techniques that apply targeted pressure to hardened areas. Therapists may also need to address inflammation and other factors that contribute to ongoing swelling.

For some people, fibrosis treatment may also include:

  • Specialized manual techniques that help loosen stiff tissue
  • Textured or quilted compression that applies varied pressure to hardened areas
  • Intermittent pneumatic compression (IPC) that uses repeated pressure to support lymph flow and soften tissue
  • Other tissue-softening therapies, such as suction (negative pressure) or low-level laser treatment

Ashforth often layers specialized textured or quilted materials under IPC to create more targeted pressure. These “anti-fibrotic garments” work well on their own, but the additional pressure of IPC can accelerate the tissue-softening effect.

Reducing swelling and softening fibrosis are critical, but Ashforth sees effective self-management as the larger goal.
